If you wish to use
the facilities owned by the Pembroke School District (including the
buildings themselves or our sports fields) you must fill out an
application and present it to the SAU office. Please follow the
rules for filling out the application closely and be sure to sign the form directly above the
"Hold Harmless Agreement" box and inside the "Hold
Harmless Agreement" box. You must also present a current
proof of insurance with your application. ~
Facilities are NOT to be used until the entire application process has
been COMPLETED ~ This
includes your two signatures, proof of insurance, a signature from
the building Principal and a signature from the Business Administrator in
the SAU office. Once the signatures have all been collected you will
receive your copy of the signed form. At this point, you may use the
facilities as agreed. PLEASE
NOTE: Where applicable, payment must also be made before the
facilities are used. Please see the rules for details
regarding fees. Below
you will find the rules and application for the use of our Pembroke
facilities. Please complete and return the application to either the
school of interest or the SAU office. RULES
